I, the undersigned, understand this information will be reviewed by the Board before my consideration for a position on the Board. I also affirm that if selected I will do my best to serve as fully as possible in the duties I assume. Board meetings are held the second Wednesday of the Month at 8:30 a.m. Meeting attendance is required. Missing 2 consecutive or a total of 3 meetings within one year is grounds for replacement on the Board. This is a two-year commitment with the option of additional terms.
